| Field Name | Description |

| Data Source | Provide the type of source you want to use (such as Microsoft Access). |
| User Name | Provide the user name to which you will log into the data source (if needed). |
| Password | Provide the password to which you will use to log into the data source (if needed). |
| Table Name | Select the table to which you want to connect. If an error pops up, you either do not have the a valid data source, user name, or password. |
